JULIET: I’ve been on this island for 3 years, Jack. 3 years, 2 months, and 28 days. He said that if I let him live and I helped you — that he would finally let me go home.

Juliet es también una prisionera de Ben y, como los losties, está atrapada en la Isla. Llegó a ella reclutada por Richard Alpert (primera aparición), que buscaba a una experta en fertilidad y le ofreció un jugoso contrato para que trabajase con él en sus instalaciones de, supuestamente, Portland.

ALPERT: Thank you for taking the time to let us make our pitch, Dr. Burke. Mittelos Bioscience is based in Oregon. We’re just outside of Portland.

JULIET: I can’t. My ex-husband wouldn’t let me.

ALPERT: There must be something that he would respond to.

JULIET: If he were hit by a bus. How about that? That would work. How totally inappropriate. I’m sorry, but I’ve wasted your time.

Juliet declina la oferta. Su jefe y ex marido, Edmund Burke (interpretado por Željko Ivanek), cuyo placer nada secreto es hacerle la vida imposible, nunca lo permitirá. Salvo, quizás, que lo atropelle un autobús, dice ella en un arrebato por el que enseguida se disculpa.

Sea porque Alpert ha tomado nota o por simple casualidad, el caso es que la siguiente vez que vemos a Edmund Burke se lo lleva por delante un autobús. Y Alpert vuelve para reiterarle a Juliet su oferta.

ALPERT: Just give us six months. If you need to, you can be back before your sister gives birth.

JULIET: Can my sister come?

ALPERT: Won’t work. We’re pretty remote. She wouldn’t have access to the treatment that she needs.

JULIET: But, it’s Portland. There’s plenty of clinics and…

ALPERT: Well, actually we’re not quite in Portland.

Mientras, en el presente, en la Isla, Ben se despierta durante la cirugía:

JACK: I stopped the surgery.

BEN: I know. I’ve been able to hear you for a few minutes now. It’s very clever of you. I should have seen it coming.

JACK: Are you in pain? I can…

BEN: No, but thanks all the same.

Y en su huida, Kate y Sawyer cuentan con una inesperada aliada (bueno, dos, porque al final Juliet les echa también una mano), una joven llamada Alex que resulta ser la hija de Ben…

…y novia de Carl, el ocupante de la jaula vecina a la de Sawyer cuando él fue trasladado allí, al que rescatan de una sesión de reeducación al estilo de La naranja mecánica:
